The following tests have full approval to be sent by providers from New York State:
- Claritas Clinical Exome – Trio and Proband Only
- Mitochondrial DNA Analysis has received Conditional Approval
- Pediatric Neurology Region of Interest – Trio and Proband Only
How to order
Providers who are ordering the tests above may use the Claritas Exome-Based Test Requisition form. For tests that are not on the list above, providers are advised to continue to use the Exemption form.
Special notes:
- Some analyses that are companion tests for approved tests have not yet been awarded full approval through New York State. To order these companion analyses (ex. the high-resolution deletion/duplication analysis), use the Exemption form.
- Instructions for requested non-permitted tests from New York State Department of Health can be found here. We recommend obtaining permission from NYS DOH before shipping the sample to Claritas.
