Genetic testing is often an expensive process, and costs can deter providers and families from obtaining necessary testing. Claritas has developed billing and financial programs to minimize expense for patients at all levels of income and insurance. In addition, Claritas works directly with familes and insurance providers to resolve denied claims and/or patient financial responsibility with respect to all applicable state and federal laws.

Institutional Billing

For hospitals or organizations in the United States and Canada that wish Claritas to bill directly, please complete the Client Registration Form in advance of sending specimens, if at all possible. An account will be created for the organization and invoices will be sent on a monthly basis.

International Samples: Claritas requires pre-payment for testing from institutions outside the United States or Canada. We accept payment by money order, bank transfer, credit card, or PayPal. Contact Claritas Client Services (Toll Free: (855) 373-9003, [email protected]) for more information.

Commercial Insurance/Third-Party Billing

Claritas Genomics can bill patient’s health insurance and will make every effort to appeal any denied claim or unreasonable patient responsibility.
Requirements for Claritas to bill commercial insurance:

  • Completed insurance information
  • Patient Signature
  • Copy of front and back of the patient’s health insurance card(s)

Patients will receive bills from Claritas for any non-covered or out of network services, co-payments, deductibles, or balances as required by their insurers unless the patient applies and meets financial criteria defined by the Claritas Patient Access Program. Self-Pay Pre-Payment

Patients may choose to pre-pay for testing and receive a 25% discount. Payment must be made prior to the receipt of the specimen. A completed test requisition is required. To pay by credit card, download the Credit Card Payment form. Claritas will coordinate with patient/family to accept money orders or bank transfers. Financial Assistance Program

Claritas Genomics’ Financial Assistance Program is established to help patients and families plan for their medical costs, whether or not they have health insurance. The program provides support to patients and families to help mitigate overall out of pocket expenses based on their needs. Patients must contact Claritas in order to discuss and determine if they qualify for this program.

Payment Plans

All patients and their families are eligible for a payment plan with the exception of those electing the self-pay pre-payment option. Patients must contact our Billing Department to discuss and enroll in this program.

Prompt Pay Discount

Patients who receive a statement from Claritas Genomics and remit payment in full within 30 days of the statement date receive a 10% discount. 

Economic Hardship Program

Enrollment into this program is initiated by the patient’s ordering provider. To be eligible for this program, patients must be receiving free services from the ordering/enrolling provider, must be uninsured, and must have a total household income below 100% of the Federal Poverty Guidelines. Providers may contact Claritas Genomics on behalf of their patients for more information. Claritas will not review applications sent directly from patients.

Cancellation Policies

Testing may be cancelled within 24 hours of receipt at the laboratory, if the sample is not placed on hold. After this time, the full cost of testing will apply. Once testing has been started, a test cannot be cancelled and full charges will apply. We recommend that Health Care Provider contact our Client Services department directly (Toll Free: (855) 373-9003, [email protected]) to cancel or hold testing. Additional Information

Claritas reserves the right to modify or discontinue any programs described in this policy. Claritas reserves the right to request supporting documentation for the Economic Hardship Program.
